8943ca11b2 qemu: prepare NVMe devices too
The qemu driver has its own wrappers around virHostdev module (so
that some arguments are filled in automatically). Extend these to
include NVMe devices too.

f988128cc1 qemu_hostdev: Introduce qemuHostdevNeedsVFIO()
There are two types of host devices that require /dev/vfio/vfio
access:

  1) PCI devices with VFIO backend
  2) Mediated devices

Introduce a simple helper that returns true if passed @hostdev
falls in either of the categories.

80b58a7c1a qemu: Drop KVM assignment
KVM style of PCI devices assignment was dropped in kernel in
favor of vfio pci (see kernel commit v4.12-rc1~68^2~65). Since
vfio is around for quite some time now and is far superior
discourage people in using KVM style.

Ideally, I'd make QEMU_CAPS_VFIO_PCI implicitly assumed but turns
out qemu-3.0.0 doesn't support vfio-pci device for RISC-V.

9f95552d17 qemu: De-duplicate some path definitions
There are some paths (e.g. /dev/vfio/vfio or /dev/mapper/control)
which are defined in qemu_domain.c and then in qemu_cgroup.c
again. This is suboptimal. Let's move paths into qemu_domain.h and
drop duplicate definitions.

When commit 1d94b3e7 added code to walk the [n]hostdevs list looking
to add shared hostdevs, it should've filtered any hostdevs that were
not SCSI hostdev's.

600462834f Remove all Author(s): lines from source file headers
In many files there are header comments that contain an Author:
statement, supposedly reflecting who originally wrote the code.
In a large collaborative project like libvirt, any non-trivial
file will have been modified by a large number of different
contributors. IOW, the Author: comments are quickly out of date,
omitting people who have made significant contribitions.

In some places Author: lines have been added despite the person
merely being responsible for creating the file by moving existing
code out of another file. IOW, the Author: lines give an incorrect
record of authorship.

With this all in mind, the comments are useless as a means to identify
who to talk to about code in a particular file. Contributors will always
be better off using 'git log' and 'git blame' if they need to  find the
author of a particular bit of code.

This commit thus deletes all Author: comments from the source and adds
a rule to prevent them reappearing.

The Copyright headers are similarly misleading and inaccurate, however,
we cannot delete these as they have legal meaning, despite being largely
inaccurate. In addition only the copyright holder is permitted to change
their respective copyright statement.

5165ff0971 src: More cleanup of some system headers already contained in internal.h
All of the ones being removed are pulled in by internal.h. The only
exception is sanlock which expects the application to include <stdint.h>
before sanlock's headers, because sanlock prototypes use fixed width
int, but they don't include stdint.h themselves, so we have to leave
that one in place.

e02d102bac qemu: hostdev: Fix the error on VM start with an mdev when IOMMU is off
Commit b4c2ac8d56 made a false assumption that IOMMU support necessary
for an mdev device to be assigned to a VM. Unlike direct PCI assignment,
IOMMU support is not needed for mediated devices, as the physical parent
device provides the isolation, therefore, simply checking for VFIO
presence is enough to successfully start a VM.

Luckily, this issue is not serious, since as of yet, libvirt mandates
mdevs to be pre-created prior to a domain's launch - if it is,
everything does work smoothly even with IOMMU disabled, because the
parent device will ensure the iommu groups we try to access exist.
However, if there are *no* IOMMU groups yet, thus no mdev exists yet, one
would see the following error:

"unsupported configuration: Mediated host device assignment requires VFIO
support"

The error msg above is simply wrong and doesn't even reflect the IOMMU
reality, so after applying this patch one would rather see the following
error in such case instead:

"device not found: mediated device '<UUID>' not found"

b4c2ac8d56 qemu: Fix mdev checking for VFIO support
Commit a4a39d90 added a check that checks for VFIO support with mediated
devices. The problem is that the hostdev preparing functions behave like
a fallthrough if device of that specific type doesn't exist. However,
the check for VFIO support was independent of the existence of a mdev
device which caused the guest to fail to start with any device to be
directly assigned if VFIO was disabled/unavailable in the kernel.
The proposed change first ensures that it makes sense to check for VFIO
support in the first place, and only then performs the VFIO support check
itself.

Keep track of the assigned mediated devices the same way we do it for
the rest of hostdevs. Methods like 'Prepare', 'Update', and 'ReAttach'
are introduced by this patch.

9cc26dc622 qemu: Add vhost-scsi string for -device parameter
Open /dev/vhost-scsi, and record the resulting file descriptor, so that
the guest has access to the host device outside of the libvirt daemon.
Pass this information, along with data parsed from the XML file, to build
a device string for the qemu command line.  That device string will be
for either a vhost-scsi-ccw device in the case of an s390 machine, or
vhost-scsi-pci for any others.

66f319aec0 qemu: hostdev: Introduce qemuHostdevUpdateActiveDomainDevices()
This calls the PCI-, USB- and SCSI-specific functions just
like qemuHostdev{Prepare,ReAttach}DomainDevices() already do,
and was the missing piece for the qemuHostdev API to nicely
mirror the virHostdev API.

Update qemuProcessReconnect() to use the new function.

2835c1e730 Add virLogSource variables to all source files
Any source file which calls the logging APIs now needs
to have a VIR_LOG_INIT("source.name") declaration at
the start of the file. This provides a static variable
of the virLogSource type.
