This patch removes the explicit FSF address, and uses above instead

Except a bunch of files for security driver, all others are changed

* src/util/threads-win32.h (includes): Pick up winsock2.h before
windows.h, as required by mingw64.
mingw lacks the counterpart to PTHREAD_MUTEX_INITIALIZER, so the
best we can do is portably expose once-only runtime initialization.
