For the bus scanning the GED AML code now calls into a PSCN method that
scans all buses. This approach was chosen since it handles the case
correctly where one GED interrupt is services for two hotplugs on
distinct segments.
The PCIU and PCID field values are now determined by the PSEG field that
is uses to select which segment those values should be used for.
Similarly _EJ0 will notify based on the value of _SEG.

This was causing some issues because of the use of 2 different versions
for the vm-memmory crate. We'll wait for all dependencies to be properly
resolved before we move to 0.7.0.

In preparation for reorganizing how the serial output is constructed
add methods to the serial devices for setting the out buffer after the
device is created.
Also add a method to enable flushing the output buffer to be used to
write the buffer to the PTY fd once the PTY is writable.

On AArch64, interrupt controller (GIC) is emulated by KVM. VMM need to
set IRQ routing for devices, including legacy ones.
Before this commit, IRQ routing was only set for MSI. Legacy routing
entries of type KVM_IRQ_ROUTING_IRQCHIP were missing. That is way legacy
devices (like serial device ttyS0) does not work.
The setting of X86 IRQ routing entries are not impacted.

Both GIC and IOAPIC must implement a new method notifier() in order to
provide the caller with an EventFd corresponding to the IRQ it refers
to.
This is needed in anticipation for supporting INTx with VFIO PCI
devices.

In anticipation for supporting the notifier function for the legacy
interrupt source group, we need this function to return an EventFd
instead of a reference to this same EventFd.
The reason is we can't return a reference when there's an Arc<Mutex<>>
involved in the call chain.

This can be uses to indicate to the caller that it should wait on the
barrier before returning as there is some asynchronous activity
triggered by the write which requires the KVM exit to block until it's
completed.
This is useful for having vCPU thread wait for the VMM thread to proceed
to activate the virtio devices.

When the destination mode is physical, the destination field should
only be defined through bits 56-59, as defined in the IOAPIC spec. But
from the APIC specification, the APIC ID is always defined on 8 bits no
matter which destination mode is selected. That's why we always retrieve
the destination field based on bits 56-63.

When created, the IOAPIC entries should be masked, as it is the guest's
responsibility (FW and/or OS) to unmask them if/when necessary.
This patch saves a full round of port I/O writes from the guest to the
IOAPIC, meant for masking the unmasked entries.
Because they're now masked, the entries are not enabled, which means
they are not connected from a KVM perspective, saving from unneeded
registration/unregistration of the irq fds.

Unlike x86_64, the "interrupt_controller" in the device manager
for AArch64 is only a `Gic` object that implements the
`InterruptController` to provide the interrupt delivery service.
This is not the real GIC device so that we do not need to save
its states. Also, we do not need to insert it to the device_tree.

This is a counter exposed via an I/O port that runs at 3.579545MHz. Here
we use a hardcoded I/O and expose the details through the FADT table.

This bit should be set for the last 224us of each second. To be able to
fill this bit we need to use clock_gettime() to get the nanoseconds.
Also set the frequency bit to indicate we update at 32kHz.

This commit adds the implementation for the AArch64 PL031
Real Time Clock (RTC) that provides a long time base counter. This
is achieved by generating an interrupt signal after counting a
programmed number of cycles of a real-time clock input. The AArch64
guest VM of the cloud-hypervisor will use this RTC to sync the time
in itself.

IOAPIC, a X86 specific interrupt controller, is referenced by device
manager and CPU manager. To work with more architectures, a common
type for all architectures is needed.
This commit introduces trait InterruptController to provide architecture
agnostic functions. Device manager and CPU manager can use it without
caring what the underlying device is.

OVMF and other standard firmwares use I/O port 0x402 as a simple debug
port by writing ASCII characters to it. This is gated under a feature
that is not enabled by default.

The same BusDevice can be inserted multiple times on the same bus at
different ranges. This is the case for a PCI device with multiple memory
BARs for example, as each BAR will be inserted at a specific MMIO range.
This new method allows to find and remove every occurence of the same
device so that we can make sure it is fully removed from the bus.

Expands the existing GED device description in the DSDT table. A new
case of hotplug notification is being added to the already existing CPU
and memory hotplug. The third case of hotplug being the addition/removal
of a PCI device, it is identified through the flag 0x4.

Having the InterruptManager trait depend on an InterruptType forces
implementations into supporting potentially very different kind of
interrupts from the same code base. What we're defining through the
current, interrupt type based create_group() method is a need for having
different interrupt managers for different kind of interrupts.
By associating the InterruptManager trait to an interrupt group
configuration type, we create a cleaner design to support that need as
we're basically saying that one interrupt manager should have the single
responsibility of supporting one kind of interrupt (defined through its
configuration).

This commit relies on the interrupt manager and the resulting interrupt
source group to abstract the knowledge about KVM and how interrupts are
updated and delivered.
This allows the entire "devices" crate to be freed from kvm_ioctls and
kvm_bindings dependencies.

Use independent bits for storing whether there is a CPU or memory device
changed when reporting changes via ACPI GED interrupt. This prevents a
later notification squashing an earlier one and ensure that hotplugging
both CPU and memory at the same time succeeds.

Since the kvm crates now depend on vmm-sys-util, the bump must be
atomic.
The kvm-bindings and ioctls 0.2.0 and 0.4.0 crates come with a few API
changes, one of them being the use of a kvm_ioctls specific error type.
Porting our code to that type makes for a fairly large diff stat.

We need to rely on the latest kvm-ioctls version to benefit from the
recent addition of unregister_ioevent(), allowing us to detach a
previously registered eventfd to a PIO or MMIO guest address.
Because of this update, we had to modify the current constraint we had
on the vmm-sys-util crate, using ">= 0.1.1" instead of being strictly
tied to "0.2.0".
Once the dependency conflict resolved, this commit took care of fixing
build issues caused by recent modification of kvm-ioctls relying on
EventFd reference instead of RawFd.

The command "cargo build --no-default-features" does not recursively
disable the default features across the workspace. Instead add an acpi
feature at the top-level, making it default, and then make that feature
conditional on all the crate acpi features.

Add an I/O port "device" to handle requests from the kernel to shutdown
or trigger a reboot, borrowing an I/O used for ACPI on the Q35 platform.
The details of this I/O port are included in the FADT
(SLEEP_STATUS_REG/SLEEP_CONTROL_REG/RESET_REG) with the details of the
value to write in the FADT for reset (RESET_VALUE) and in the DSDT for
shutdown (S5 -> 0x05)

Add a 2nd EventFd to the VM to control resetting (rebooting) the VM this
supplements the EventFd used for managing shutdown of the VM.
The default behaviour on i8042 or triple-fault based reset is currently
unchanged i.e. it will trigger a shutdown.
In order to support restarting the VM it was necessary to make start()
function take a reference to the config.

Latest clippy version complains about our existing code for the
following reasons:
- trait objects without an explicit `dyn` are deprecated
- `...` range patterns are deprecated
- lint `clippy::const_static_lifetime` has been renamed to
`clippy::redundant_static_lifetimes`
- unnecessary `unsafe` block
- unneeded return statement
All these issues have been fixed through this patch, and rustfmt has
been run to cleanup potential formatting errors due to those changes.

Poor performance was observed when booting kernels with "console=ttyS0"
and the serial port disabled.
This change introduces a "null" console output mode and makes it the
default for the serial console. In this case the serial port
is advertised as per other output modes but there is no input and any
output is dropped.

When resolving an IO address to a device, return the range base address,
the offset, and the device itself.
This is needed for devices with multiple IO regions to find out which
region an IO/MMIO exit is coming from.
We also use this change as an opportunity to rename get_device to
resolve as we're doing more than just getting a device.

The goal for cloud-hypervisor is to keep the host safe. With this in
mind, we want to emulate as much as possible in userspace instead of
in kernel directly.
The IOAPIC is a good candidate to move from kernel to userspace, which
is why this commit introduces a userspace implementation of the IOAPIC
82093AA based on the documentation:

This commit anticipate the future need from having support for both
in kernel and userspace IOAPIC. The way to signal an interrupt from
the serial device will vary depending on the use case, but this should
be independent from the serial implementation itself.
That's why this patch provides a generic trait for the serial device
to call from, so that it can trigger interrupts independently from the
IOAPIC type chosen (in kernel vs userspace).
